Self Assessment payment slip

HM Revenue & Customs (HMRC) recommend that you pay electronically. This is safer, more secure and very efficient. If you pay using one of the following methods you do not need your HMRC payslip:

  • Direct Debit
  • Debit or credit card over the internet using BillPay
  • Bacs Direct Credit, internet or telephone banking or CHAPS

If you choose to post your payment to HMRC please use the HMRC computer printed payslip as this contains all your preprinted information.

If you do not have your computer-printed payslip, please complete the boxes below then click the 'Submit' button, this will produce a printable copy of this form. Print this off and send it with your cheque payment.

Make your cheque payable to 'HM Revenue and Customs only' followed by your/your client's Self Assessment reference number, also know as a Unique Taxpayer Reference (UTR).

If you are a new Self Assessment customer and do not have a UTR please provide your/your client's National Insurance number instead.

Send the payment slip and your cheque to HMRC (in the return envelope, if provided). A stamp for the correct postage is required.
